Remote Monitoring and Care Service
Remote monitoring and care is one of the ways we are supporting residents with ongoing health conditions to keep well at home. Instead of waiting to contact your healthcare professional, they can now proactively support you using clinical expertise, your shared care record, and remote monitoring data.
The Remote Monitoring and Care Service is coordinated by East Berkshire Primary Care and Berkshire Primary Care, who each have Digital Healthcare Teams. If you receive a text from them, it means we believe this service could benefit you and they will contact you soon to explain how it works and answer any questions you might have.
What Is Remote Monitoring and Care?
You’ll regularly share health readings, like blood pressure and oxygen levels, and answer questions about your health and wellbeing via a remote monitoring app on your smartphone or tablet. Healthcare professionals at the Remote Monitoring and Care Service review this health information and offer clinical advice and support when you need it.
New Remote Monitoring App
We’re introducing a more user-friendly app developed by Luscii to replace the current DOCOBOAPP™. From November 2025, new users will use the Luscii App. Existing
DOCOBOAPP™ users will be given guidance on how to switch when it is time to change over.
How We Use Your Health Information
Your health information is securely collected and monitored to support your care. Only relevant healthcare professionals will have access to your information. All data is stored securely following NHS confidentiality and data protection policies. We will not share your data with third parties outside your care team without your consent unless required by law.

What Happens Next?
You will receive a call to discuss the service. If you would like to be enrolled, the relevant Digital Healthcare Team will work with you to understand what equipment you might need and give you guidance on how to set up the app and share your health readings.
