Travel Vaccinations and Non-NHS Services

If you require any vaccinations relating to foreign travel you will need to make an appointment with a nurse. This initial appointment must be at least 6 weeks before you travel.

Before attending your first appointment, please complete this Travel Questionnaire and submit it.

The NHS does not pay for all of the services we provide, so patients may be required to pay for some of these services. Our fees are in line with BMA approved national guidelines. Please see attached list.

Local Secondary Care Services

We sit across the Berkshire/Surrey border so are able to offer Heatherwood and Wexham Park, Frimley Park and St Peter’s Hospitals. If your GP feels a Consultant’s opinion is needed, your choice of hospital will be discussed with you. You are entitled to ask for a copy of the referral letter.

Child Protection

One of the responsibilities passed onto your Doctor’s team by central government and the General Medical Council is to ensure the safeguarding of children who attend this practice. If we have child protection concerns regarding your child/children we will always endeavour to discuss this with you first.

Health Visitor

The Health Visitor is a specialist nurse concerned with promoting and monitoring the health of families with particular reference to the under fives. Contact telephone number for the Health Visitor is 01753 636523.

District Nurses

District nurses are specially trained to provide nursing care in the home for those unable to attend the surgery. They can tell you what help is available and arrange for any equipment or supplies that may be needed. They are able to provide information on the availability, and means of acquiring support such as financial help, allowances, voluntary & statutory services, social services, etc. Their contact telephone number is 0300 365 1234.

The Citizens Advice Bureau will also be able to help with information regarding financial support. They run a drop-in session at The Chandler Centre, Ascot Day Centre, Sunninghill on Tuesdays from 10:00 until 12:00.

Midwife

The midwifery team hold a clinic at Magnolia House every Tuesday afternoon from 13.30 until 15.30 for all our pregnant ladies regardless of where you have chosen to deliver your baby. Please telephone reception to book an appointment.

Practice Nurses

Our nursing team is available every day to help with health checks, immunisations, dressings, travel advice, smoking cessation, blood tests, suture removal and many more aspects of health care and prevention. The Practice is an accredited Yellow Fever Centre. Specialist nurses are available to deal with chronic diseases such as Asthma, Diabetes and Coronary Heart Disease.
